Medical bill + EOB comparison worksheet

A provider bill asks for payment. An explanation of benefits (EOB) explains how a health plan processed a claim and is usually not a bill. Use this sheet to compare one bill with one EOB.

3. Mark what needs an answer

Names, service dates, or locations do not match. The service description is unclear. The bill may have arrived before claim processing finished. An itemized bill is needed. The plan says information or authorization is missing. A duplicate or corrected claim may exist. A deadline needs to be added to the calendar. Financial-assistance or payment-plan information is needed.

Before paying a balance you do not understand: confirm that the bill and EOB refer to the same claim, ask the sender about any deadline, and keep copies of every notice. This worksheet cannot determine whether a charge is correct, covered, or legally permitted.
