PATIENT INFORMATION

Provider directory sources and corrections

CareFinder is a planning directory, not a verified appointment service or an insurer’s network directory. Use a result as a lead to confirm directly, not as proof that a location provides the care you need.

What the imported information means

The directory includes imported NPPES/CMS registry records and other ingestion sources. Source information is not complete for every field. An NPI identifies an enumerated individual or organization; it does not establish current licensure, a clinic’s services, walk-in availability, insurance participation, or the accuracy of every address. A legacy field labeled CMS verified is not a clinical or insurance endorsement.

Care categories in search results are imported classifications and can be wrong. NPPES taxonomy describes a reported provider type, not a promise that a particular office offers that service today. A category identified as inconsistent is withheld pending review; we do not automatically reclassify the office or delete its source record.

How urgent-care categories are checked

When an imported record contains taxonomy codes, the urgent-care category requires code 261QU0200X (Clinic/Center, Urgent Care). If its known codes do not include that classification, the listing is withheld from public results pending further confirmation. That can exclude a legitimate practice until its service is documented; it does not mean the practice is closed or cannot provide urgent appointments. Records without taxonomy codes remain unconfirmed, not verified by this check.

Distance and completeness limits

ZIP searches use an approximate ZIP center. Distances are straight-line estimates, not driving directions or travel times. Records with recognized legacy city-center coordinates, invalid coordinates, or explicit public-visibility exclusions are omitted. Address-level geocoding does not establish that a facility is open.

Searches return at most 20 results from a bounded set of nearby candidates. Filtering and query limits can leave other suitable offices undisplayed, including closer offices. No result does not mean no care exists. The directory does not show live operating status, a verified last-contact date, or a nationwide completeness guarantee. If a field lacks a documented confirmation date, treat it as unconfirmed.

Confirm before non-emergency care

Call the facility to confirm its exact address, services, ages treated, appointment rules, hours and official website. Call your insurer using the number on your card to confirm the clinician, facility and service under your exact plan. A listing or NPI cannot determine coverage. For a possible emergency, call 911; do not wait for directory or network checks.

Request a listing correction

On a provider’s detail screen, choose Report a listing issue. Include only the public listing reference, the field that appears wrong, and a public source or a way to confirm it. You may also email info@carefinder.info. Do not send medical records, symptoms, member IDs, financial information, or another person’s private details. Reports require review; submission does not guarantee an immediate edit.
